Enhancing male fertility with MESA and TESE

At Thomson Fertility Centre, we understand that male infertility can present unique challenges. Some men, due to factors like prior vasectomy or obstructions in their spermatic ducts, may not have sperm present in their ejaculate.


Our fertility specialists offer advanced surgical procedures known as micro-epididymal sperm aspiration (MESA) and testicular sperm extraction (TESE) for assisted reproduction. As outpatient procedures, you can typically return home the same day; recovery is generally quick, with most patients able to resume normal activities within a few days.

Microscopic Epididymal Sperm Aspiration (MESA)

In MESA, sperm are retrieved from the epididymis, a coiled tube located on the back of each testicle where sperm are stored and mature. This procedure specifically targets the epididymal region.


MESA is often chosen when a man has a vasectomy reversal or a blockage in his spermatic ducts that prevents sperm from reaching the ejaculate. It's a preferred method when epididymal sperm are accessible and viable, as the sperm retrieved is generally of better quality after maturing within the epididymis. For the patient, recovery is often quicker with less post-operative discomfort.

Testicular Sperm Extraction (TESE)

TESE involves the extraction of sperm directly from the testicle. The surgeon makes an incision in the testicle and removes small tissue samples, which are then examined for the presence of sperm.

TESE is typically performed when there is a more significant issue with sperm production or maturation within the testicles. It's used when sperm are not present in the epididymis or when MESA is not feasible due to anatomical factors. Sperm obtained through TESE may have lower motility and maturity levels because they are extracted directly from the testicular tissue.



Patients may experience slightly more discomfort post-surgery due to the larger testicular incision, but our team will thorough post-procedure instructions on managing the discomfort, including guidance on wound care and any necessary follow-up appointments.
