Everyone's journey to parenthood is unique, but if you're struggling with unexplained infertility, it's only natural to consider other fertility options, such as assisted reproductive technologies.
While looking for the best fertility treatment option, you may have come across EVIE IUI. But what sets EVIE IUI apart from standard intrauterine insemination (IUI), and is it the right choice for you?
What is EVIE IUI?
EVIE IUI, also known as Slow Release Intra-Uterine Insemination (SRI), uses a small, single-use mechanical pump (“Evie”) to slowly release prepared sperm into the uterus over four hours via a balloon catheter and an insemination syringe.
Unlike standard IUI, where sperm are injected in one go, EVIE mimics natural conception by slowly and gently releasing them, which maximises the opportunity for the sperm to reach the egg and achieve fertilisation.
What is the difference between standard IUI and EVIE IUI?
Standard IUI | EVIE IUI (slow release) | |
---|---|---|
Sperm release | Injected all in one go | Gradually released over four hours |
Device | Simple catheter and syringe | Single-use pump with balloon catheter |
Timing | Takes minutes | You will need to wear it for four hours |
Pregnancy rates (under 35) | Up to ~10% per cycle | Doubles pregnancy rate vs. standard IUI (up to ~20% per cycle) |
Approach | Delivers sperm quickly | Mimics natural sperm migration |
Patient comfort | May cause brief cramping | Mild cramping, generally well tolerated |
Should I get an EVIE IUI instead of a standard IUI?
EVIE IUI may be recommended as a first line for couples if one or more of the following apply:
A couple with unexplained infertility
Your male partner has mild male subfertility
You have endometriosis (grade 1 or 2) and have intact fallopian tubes
You has cervical mucus problem
Your male partner has anti-sperm antibodies
Ejaculation is not possible due to erectile dysfunction or vaginismus (involuntary vaginal muscle tightening).
Retrograde ejaculation (where the semen travel backwards in the bladder)

How can I prepare for an EVIE IUI?
b\The preparation for an EVIE IUI procedure will be similar to the preparation for a standard IUI procedure. You will need a thorough medical exam and fertility tests. Your partner will be examined and tested as well. This could include:
A uterine exam
Ultrasounds of your uterus
Screening for sexually transmitted infections (STIs) and other infectious diseases
Blood tests
Your healthcare provider may recommend taking folic acid (included in most prenatal vitamins) at least three months before conception (or IUI treatment).
Remember to also:
Attend all pre-treatment appointments:
Your fertility specialist will assess your suitability and discuss timing.
Follow medication instructions:
Ovulation-inducing drugs may be prescribed, as with other IUI cycles.
Dress comfortably:
During EVIE IUI, the device is strapped to your thigh for four hours.
Plan light activities:
You can carry out light duties (walking, light household tasks), but avoid vigorous movement.
What is the process of getting an EVIE IUI like?
Upon your arrival, the sperm will have already been prepared, a process that typically takes around an hour. The actual EVIE procedure lasts about 15 minutes and is similar to a standard IUI or routine exams like a smear test. It is noted for being less invasive than treatments such as IVF or ICSI.
A thin catheter is gently inserted into the uterus, making it a quick and straightforward process. You might feel mild cramping, similar to regular IUI. Your doctor might recommend a mild pain reliever to ease any discomfort.
Once the EVIE device is positioned, the doctor will record the "start time" and "end time" and guide you through the simple removal process after four hours. You will receive a copy of these times along with removal instructions.
The EVIE procedure is designed to be simple, fast, and convenient. After the device is set and activated, you can leave the clinic. Although you can drive and walk, it is advisable to relax comfortably for the four-hour duration for optimal results.
Avoid making sudden vigorous movements with the device or catheter during the process. Bathroom visits are allowed, but it's best to avoid bowel movements if possible to reduce infection risk. Physical exertion and intercourse should be postponed until after removing the device. The doctor will offer detailed instructions and provide you with a guide for the removal process.

How much does an EVIE IUI cost?
The costs may vary depending on the clinic, medications, and medical examinations required.
In private fertility centres, the cost of a single standard IUI cycle is higher, typically ranging from SGD 1,800 to SGD 3,000. This amount usually includes doctor consultation fees, injections, ultrasound scans, hormonal medications (hCG), and blood tests.
Subsequent cycles within six months may cost around SGD 700 to SGD 900. Some clinics offer more affordable packages starting at SGD 880 to SGD 1,000, but these often exclude additional costs for medications and medical exams.
Additional costs for diagnostic tests such as semen analysis or hormone testing can add SGD 96 to SGD 600 depending on the complexity of the tests required. Medications and ovulation induction injections may also increase the overall cost by up to SGD 2,000 per cycle.
The costs for an EVIE IUI should be similar to what is mentioned above. However, it is best to discuss directly with your healthcare provider for the most accurate pricing.
Financial assistance options
MediSave
Couples can use Medisave to offset IUI costs. Withdrawal limits are:
SGD 6,000 for the first cycle

Government co-funding
Singaporean couples undergoing IUI at public hospitals may qualify for government co-funding if they meet specific criteria:
Couples below 40 years old
Assessed by a doctor as suitable for IUI before IVF
At least one spouse is a Singaporean citizen
You have not previously received 3 co-funded fresh and 3 co-funded frozen cycles
Co-funding covers up to three cycles based on the citizenship status of the couple:
75% (up to SGD 1,000) for Singaporean couples
55% (up to SGD 700) for Singaporean and PR couples
35% (up to SGD 500) for Singaporean and foreigner couples
The prices mentioned above reflect current policies as of July 2025. However, please note that the prices of both IUI and the government co-funding are subject to change. FAQ
Are there any risks to an EVIE IUI?
The risks of an EVIE IUI are similar to that of a standard IUI, which includes mild cramping and a very low infection risk. Slow release may reduce immunological reactions and lower the chance of polyspermia (too many sperm fertilising one egg), making it potentially safer in certain ways. Severe complications with EVIE IUI are rare.
Can I immediately go back to my normal activities after an EVIE IUI?
Yes, light activities are permitted during the four-hour insemination period. Avoid strenuous exercise, sudden movement, or intercourse on the day of the procedure. Most people can resume daily routines right after finishing the treatment.
What are the advantages of an EVIE IUI?
The advantages are:
There is a significant increase in pregnancy rates in younger women, with the rates being double those of standard IUI.
It mimics natural conception more closely.
The procedure is well-tolerated and doesn't necessitate a hospital stay.
It is ambulatory, meaning you can move around and even leave the clinic during the process.
What are the disadvantages of an EVIE IUI?
The disadvantages are:
There is a slightly higher cost for EVIE IUI compared to standard IUI.
There is limited availability on where you can get an EVIE IUI, as it is currently offered at select centres.
EVIE IUI requires wearing a device for four hours and avoiding vigorous activity during that time.
Can you pick gender with IUI?
No, IUI, including the EVIE technique, cannot reliably select the gender of a baby. Gender selection requires more advanced technologies available through IVF with genetic screening.
What is the success rate of IUI?
For standard IUI, the success rate is about 5–10% per cycle for women under 35 years old, and it decreases for women over age 35.
For EVIE IUI, the success rate is up to twice that of a standard IUI at about 17–20% per cycle for women under 35. Success rates fall with age and vary by personal fertility factors.
